If (as suggested by hhinner) you apply for a second 30-day extension, have it refused and are given 7 days to leave the country you don't get a 'black mark' against your name as it will have been done within the rules, as far as I understand it.

You do get a big "extension refused" stamp that says you need to leave in 7 days. I have a bunch of 1-4 days overstay stamps and an extension refused stamp. Never had a single problem or question, including when upgrading to a different visa type.
