Can anyone translate any of this document?

My Doc at HH Hospital scheduled a procedure. Gave me pre-procedure written instructions. In Thai. Said they don't have it in English.

Today I'll make copies, then ask 3 Thais for the favor of translating it. I'm also asking the collective knowledge that is HH Forum. Photos of each half of the page attached. Big versions on Dropbox:
