Some special accounts in Thai banks allow withdrawing anywhere and at any other bank atm free of charge. One example is krungsri's Jad Hai savings account. From their website:

"Free! withdrawals fee at any ATMs nationwide that support visa chip technology and atm plus.
Free! balance inquiry fee at any ATMs nationwide that support visa chip technology and atm plus."

I can confirm that it works as advertised.

Yesterday, for the first time ever, my ATM fee was 220 bt. This was at a green Kasikornbank ATM. I suspect all the other banks will shortly be charging 220 bt, if they aren't already. Luckily, the card I use is with a USA investment firm that reimburses the ATM fee back into my account within a couple days.

I have a Citibank Thai card that allows me to withdraw funds at any bank in Thailand and no fee is charged. I can withdraw up to 50,000 baht daily.

Set up an account with Bangkok Bank then withdrawals in area are free. You can transfer money from U.S. via Bangkok Bank of New York. I don't know if they have similar arrangement from UK. I make my monthly transfer from US bank account to Bangkok Bank of NY and it is automatically transferred to my Bangkok Bank account in Thailand.

Its the simplest way I have found to get money over here from farangland.

Most of the posters seem to be using cards from their own country. Why do you not just open a Thai bank account and transfer in your money that you think you will need? No charges for ATM withdrawls if it is your own bank machine. I have Bangkok bank 150,000 per day, Kasikorn 200,000 per day, Krungsi 40,000 but I know I can upgrade it. Annual fees of between 150-700 bt apply. Drawing cash you have to give notice if very large amounts. Bank policy varies on that.
