Yes it is done 1st visit if you provide everything they ask for, if you do not have something it will require a second visit. My last extension I had my bank letter with a date 1 day before my bank book update and immigration would not accept so I had to go back to the bank for a new letter. There a second visit to immigration.

Sorry, Steve. The wording of your post suggested that a second visit was normal.

In an extension 2 years ago, I needed another document, I asked if the bank statement would be still good for next day. I was told that it's good for a week. I've done two extensions of stay based on retirement so far - never taken a map and never been asked for one. As far as I know, you should only need a map if you're doing an extension based on marriage.
I took my passport, my daughter's blue book, letter from bank, bank book, 1 passport photo and the completed form. I let the photocopy lady make copies of all the documents/pages she thought necessary.
I was in and out in 25 mins.

Can I get a retirement visa straight off a 30 day entry visa, or do I have to get a non immigrant O from abroad?

You can upgrade your entry stamp to a non O within Thailand but you must have 15 days of your entry left and be in the financial position (65K income/800K in Thai bank or combination) to do so.
You'll get the visa and then within the last 30 days of that permission to stay, you go back and prove financials again and get the extension.

Great Forum and just to add more to the mysteries surrounding immigration requirements and the retirement visa.
I have been on a retirement visa living in Hua Hin now for coming up to 6 years, and today I just popped into the main Immigration Office to pick up a letter to register a new vehicle. To my surprise I had to complete two sets of forms, supply more passport photo's, passport copies, house blue book copies, most of this had never been the case before just to get such a vehicle registration letter. When I enquired why so much paper this time to get a single letter I was told that my address in Thailand had not been officially registered within the back of my passport. It would appear this was a new law passed within the last two years, so it would seem it will be normal now to find at the back of one's passport two fold out pieces of scrap paper, these being one for the 90 day extension of stay validation, and the other will be for the notification of your address in Thailand. I must confess that all of the staff at the Immigration were extremely helpful and seemed so happy to take care of these matters in about 30 mins. However, I remain completely baffled why this drop off was not been picked up before during the last two renewals/extensions done at the same Immigration Office, or during the many times I have passed through BKK over the past 24 months. I can only suggest that everybody who is residing in Thailand on the retirement visa should check to make sure they have these two pieces of folded up paper stapled into their passport. Nevertheless, I remain happy and content with the service offered today, to have contributed THB 500 to the Immigration service and to witness this main office for the first like a ghost town. Possibly why everybody was so happy and very helpful. Not sure if others have experienced this, or its just me missing something important again.

^ Yes, it's called the "24 hour report" you are supposed to report to immigration and fill out the necessary forms within 24 hours of arriving in Thailand, or even within 24 hours of arriving back at your house from a trip within Thailand when you stayed/registered at a hotel or other place that reported your location electronically to immigration HQ. It's pretty much in place all over the country now and appears it will be a permanent fixture, even though many of us thought it would live a short life and disappear. Just another insult a foreign long term resident is forced to endure. Pete
Edit: As a side note, having a Yellow Book residence registration with your local town government makes no difference, you still have to do the immigration 24 hour report.

I know this rule has been around for years, but I think I only ever reported to Immigration within 24 hours of arrival during my first year here and have never been questioned about not doing it.

At the airport bus station there is a new (to me) notice from immigration reminding travellers about the 24 hour visa requirement. It's pinned against the refreshment kiosk.

Not a visa but a permission to stay stamp.
Thought I had it all together today, passport copy, landing card, last permission to stay stamp and Apostille on my marriage certificate (new requirement last year, long story).
The last permission to stay copy has been sufficient for many years, this year it appears that they now require every permission to stay in the passport.
I felt like asking should I use my now defunct passport and copy the other 7 years already had? but desisted.
However, went to photo copy woman to confronted with 50 to 60 men with little green passports trying to get a photocopy. Also 2 Americans trying to start a TM7 from scratch?
Stood tall and eventually got the requed copy, the Immigration officer called us forward at next opportunity.
!0 mins and were out all hail to the lady Immigration officer who called us forward.
Point of this story is copy all extension and stamps when dealing with HH Immigration

I just leave it to the brilliant photocopy lady. She knows what the guys inside want, and I've never known her get it wrong yet.

Did mine 3 weeks ago and didn't need to copy previous extensions.
Last extension: Yes....and a copy of the non.imm visa (stamped USED) on which the extension is based.
Since the imm officer normally in charge of one year extensions doesn't like me......she made a mistake more than 10 years ago and I mentioned it to her, and she had to admit, that she was wrong...She has never forgiven me.
So whenever I go to get my extension, I make sure, that I have all my ducks in a row.
This time of course I ended up at her desk again....Not a word, she just took my papers....would have loved to find something wrong...Only words uttered from her during the 5 minutes at her counter "Look at the camera"
I brought:
Copy of the main passport page
Copy of the latest extension
Copy of the original non.imm O visa
Copy of arrival card
Bank letter
Copy of the latest page in the bank book
Copy of the first page of the bank book (showing account no. and name)
Bank book itself
Drawing of my residential address
Copy of yellow book
Yellow book itself (never been asked for it, but better safe than sorry)
Of course you have to sign all the above copies.
With the paperwork in order, the time in front of the officer will only take a few minutes.
