Tourist visa question from UK

I am filling out the Tourist Visa application - and am unsure what to put where it says: Dates of previous visits

I was in Thailand on a student visa from September 2015 - August 2016 - trying to learn Thai.

I went every 90 days to the Immigration office and had the visa extended. I also had to travel home for a few days in April 2016 - so I exited Thailand / re-entered 8 days later.

Do these Visa extensions count as visits? (as they have a stamp associated with them)
Does the re-entry count as a visit?

I have 3 pages of stamps - looks mad.

My original idea is to put down where it says: Dates of previous visits: September 2015 - as this was the EDU visa date - is that correct ?

I'd just put down the dates of your entries into Thailand. The extensions don't count, IMO.
