Where have you seen the requirement for a medical form please?

There's no need for a medical form when applying for an annual extension which I think London Boy is referring to. Ie, you make the application at HH Imm.
However, if a non Imm OA visa is being referred to (which is commonly referred to as a retirement visa), then the medical certificate is needed together with police report. The OA visa need to be applied for in your home country.
The annual extension is not a visa.
