Seriously, just keep driving through Khao Takiab on the main road towards Suan Son. It's quite a big place on the right. Being tea total these days, I can't comment on what is sold.
Coming from your place, I guess you'd turn right at the vet hospital. When you get to the main Takiab road turn right - its a few hundred yards on the right.
According to one of the local free papers he is situated opposite the turning for Anantasila Hotel and sells draft beer and Guinness but not sure if the Guinness is draft. Ring him on 092 791 7879.

Does it have a sign (in English) outside? I've gathered info for the map on 4 separate occasions, and have no reason for omitting it other than I didn't see it. Do you have a photo to help me recognise it next time please?

That is correct. It is at the intersection of the "main Khao Takiab road" and (one of) the small road that cross the railway and go up to Phetkasem road. Soi 97 when the road reach Phetkasem, I think.
A few years ago I think the place was called Uno's.
